What hospital do you refer to?

Our physicians are on staff at Riverside Methodist Hospital. We will work with each patient to find the most convenient and appropriate referrals within the OhioHealth family of healthcare providers.


What do I need to bring to my appointment?

Whether you are a new patient or a returning patient, you will find a list of what to bring to your appointment on our Visit Information page. Here you will also find the patient forms for you to fill out in advance and bring with you to your appointment.

What is your cancelled appointment policy?

If you must cancel your appointment, we’ll be happy to reschedule at a more convenient time. We ask you to give us 24 hours advance notice, providing an opportunity for another patient to been seen.


What are your billing requirements?

Patients must pay any co-payments at the time of their visits. Patients are responsible to pay any amounts, such as deductibles, co-payments and co-insurance that their insurance company designates is their responsibility. For more information, please visit Billing Information.


What should I do if I have a question about my bill?

What do I do if my prescription runs out before my next office visit?

You can ask your pharmacist to contact our office to fax a refill request to our pharmacy fax line. If your prescription is expired, you may be required to first schedule an appointment with your physician at the OhioHealth Primary Care, Endocrinology & Pulmonary Physicians before we can refill your medications.


What should I do if your office is closed and I need medical care?

If you are experiencing what you believe is a medical emergency, please call 9-1-1 or go to the closest hospital Emergency Department. When the OhioHealth Primary Care, Endocrinology & Pulmonary Physicians is closed, you will reach the answering service secretary. Visit the Contact Us page for more information.


Do you have financial assistance plans?

As part of OhioHealth, our practice is pleased to offer charity care and financial assistance to patients with limited resources and inadequate medical insurance coverage.

How do I give a family member access to my OhioHealth MyChart?

If you would like another person to have access to your OhioHealth MyChart account, please complete this proxy form and bring or mail it to our office.
