Accepted Insurance and Financial Assistance
Our physicians are providers for most of central Ohio's major health
insurance plans.
Please check with your insurance company prior to your appointment to make sure it includes OhioHealth and to learn about your plan’s covered services, payment terms and referral policy.
Please bring your current insurance card with you to your appointments.
As a courtesy, we complete insurance forms and bill insurance companies directly. Patients are responsible for co-payments, which are expected to be paid at the time of your appointment.
For your convenience, our office accepts MasterCard, Visa, Discover, personal checks and cash as payment for services.
If you are unable to show proof of health insurance coverage, we expect payment for services at the time of your office visit.
Financial Assistance/Charity Care
As part of OhioHealth, our practice is pleased to offer charity care and financial assistance to patients with limited resources and inadequate medical insurance coverage. The OhioHealth Financial Assistance/Charity program is a sliding scale based upon income and family size from 1 to 400% of the Federal Poverty Guidelines (a sliding scale discount). We also provide care for patients without health insurance at a 35% discount. Payment is due at the time of service.
